This table has been created as a guide to assist customers on what forms may need to be lodged for dealings involving a community titles scheme.
It sets out the non-plan forms used in community titles dealings and shows whether they are to be lodged as documents or evidence.

Dealings Involving a Community Titles Scheme
- New schemes
- Amendments to schemes
- Terminations
- Written consent of all type 1 and type 2 interest holders (to the subdivision of the parcel or lot by the community titles scheme) (or consent endorsed on the Notice to type 1 and type 2 interest holders for consent to subdivision and schedule of unit entitlements)
- Written consents from type 1 and type 2 interest holders and/or other registered interest holders (for purposes other than to the subdivision of the parcel or lot by the scheme, e.g. creation of s.136C TLA easement)
- Duplicate Certificate of Title (if any)
- Completion of the Statement to deal with land section on the form, if applicable
- Written consent of owners of special lots are required when making, amending or repealing exclusive use by-laws. Where all lots in the scheme are special lots, the owner will be the community corporation.
- The community development statement or consolidated version of the community development statement incorporating the numbered amendment as relevant, endorsed with the approval of Planning Commission.
- The community development statement or consolidated version of the community development statement incorporating the numbered amendment as relevant, endorsed with the approval of Planning Commission.
- Certificate of community corporation – Amendment of scheme plan effecting subdivision
- Consent statement – Type 1 and type 2 interest holders for amendment of scheme plan and schedule of unit entitlements (attaching written consents or consents affixed to the Notice to lot owners and others for amendment of scheme plan)
- Written consent by an owner of any lot proposed to be affected by the subdivision (or consents affixed to the Notice to lot owners and others for amendment of scheme plan)
- Completion of the Statement to deal with land section on the form, if applicable
- The community development statement or consolidated version of the community development statement incorporating the numbered amendment as relevant, endorsed with the approval of Planning Commission.
- Consent statement – Type 1 and type 2 interest holders for amendment of scheme plan and schedule of unit entitlements (with written consents or consents affixed to the Notice to lot owners and others for amendment of scheme plan)

- Written consent of owners of special lots when making, amending or repealing exclusive use by-laws. Where all lots in the scheme are special lots, the owner will be the community corporation.
- Written consent of Planning Commission or Local Government (as relevant) when amending or repealing by-laws under planning (scheme by-laws) condition
- Written consent from Planning Commission
- Certificate of community corporation - Schedule of unit entitlements / leases / easements/ restrictive covenants; or
- Order of State Administrative Tribunal (SAT)
- Copy of Schedule of unit entitlements acknowledged by the community corporation’s execution on the form

- Written consents of owners of affected lots
- Consent Statement - Type 1 & type 2 interest holders of land leased as temporary common property, and amendment of scheme plan to create or discharge an easement or restrictive covenant (with written consents or consents affixed to the Notice to lot owners and others for amendment of scheme plan)
- Written consent of Planning Commission for creation of short form easements or restrictive covenants
- For leases - Surrender (S) or Blank Instrument form (AE)

- Certificate of community corporation - Schedule of unit entitlements / leases / easements/ restrictive covenants
